DIABETES EDUCATION PROGRAM
Diabetes is a serious condition that can lead to a number of potential health problems. It is essential that people with diabetes are given the proper tools so they can live a healthy life. The Diabetes Education Program at Delray Medical Center can help meet the needs of patients with diabetes and pre-diabetes. This program meets the National Standards for Diabetes Self-Management Education endorsed by the American Diabetes Association.

Our Team
The Diabetes Education Program provides important medical information from certified Diabetes Educators and a team of multidisciplinary health professionals, including Registered Nurses and Registered Dietitians. Patients also have access to an array of affiliated Delray Medical Center specialists in disciplines such as wound care, podiatry and cardiovascular health.

Classes are held at Fair Oaks Pavilion on Tuesdays & Wednesdays. Education is covered by Medicare and most insurance companies. A physician’s order or prescription is necessary for admission to the program.
